Month, date, and year.
Fluent Languages
Any languages your OC can speak.
You can also include extra information, like where/when they learned to speak their second language, and what their accent is like.
Religion
Your OC's religion, or lack thereof.
Sexuality
Your OC's sexuality.
Species
This can be omitted, unless your OC belongs to a fandom where a character can be something other than human.
Occupation
Their job information goes here. I also usually include any degrees my OC has here.

Hair Color and Style
Length, color, style, etc.
Eyes
Eye shape and eye color.
Skin Tone and Complexion
Your OC's skin tone (olive, medium-fair, dark, etc.) and complexion (even, freckly, etc.).
Height, Weight, and Figure
Height and weight in whichever measuring system you prefer, and figure (hourglass, pear-shaped, etc).
Scent
Any special scented soaps or shampoos your OC uses, or just their scent overall.
Clothing Style
You can either describe, or put pictures. Or both. Whatever floats your boat.
Scars, Piercings, and Tattoos
Any scars, piercings, or tattoos your OC has. Birthmarks can also be described here.

What your OC's personality is like overall.
Just as an aside, if you have no clue how to begin coming up with a personality, try going back to his/her birth date and looking up the personality for that zodiac sign. That has always helped me out a lot.

Anything that your OC is very good at.
Shortcomings
Anything that your OC isn't very good at.
